AUTHORIZATION OF WORK

I understand, am the owner/authorized representative/tenant of the premises at which the work mentioned is to be done. I hear by authorize you to preform Recommendation, and to use such labor and materials as you deem advisable. A monthly service charge of 1.5% will be added after 30 days. I agree to pay reasonable attorney’s fees and court costs in the event of legal action. If my check does not clear, I realize I could be liable for 3 times the amount of the check, in no case more than $1,500 and in no case less than $100 all set forth in Oregon Revised Statue 30.701, plus the face value of this check and court costs. I have read this contract, including the terms and conditions available below hereof and agree to be bound by all of the terms contained herein. I have received a copy of this contract and Notice to Owner. All parts will be removed from premises and discarded unless otherwise specified herein. Credit card/Debit card purchases of $1,000 or more are subject to a 3.25% processing fee.
